Simply fill out and download our “Letter of Authorization for the Request of Historical Usage Information” provided below. For more than 7 service addresses, you will need to download Page 2 of the document here. Print the document, complete, sign and return to:

If you are requesting a quote for electricity service at your business, your signed authorization is a requirement for the release of historical usage information from your current transmission and distribution service provider (TDU or TDSP). Please be sure to list all service addresses and corresponding ESI numbers on this form. Your ESI Number (found on your billing statement) is your unique identification number for service provided at the service address listed.
Once your Letter of Authorization has been received, an Alliance Power Customer Service Representative will contact you using the contact information provided by you on this form. It’s that easy!
